Filth also marks the final film appearance of David Soul, in a cameo role.

The film is set in Edinburgh. In the film, a misanthropic Detective Sergeant is investigating a murder case, while his mental health deteriorates following the end of his marriage. He is suffering from bipolar disorder, feelings of guilt over the death of his brother, a newfound need to cross-dress as his wife, and (eventually) suicidal ideation.
